I've noticed  in other areas it knows if the accelerator has been touched at all. Even if its not enough for the car to move off.  But it might be enough for the brake to release.    Could you have a loose carpet?   

On mine the brake hold only activates when the car comes to a complete stop. Any chance  ,like I have done in the past with the handbrake  , you jumped the gun in assuming the car had fully stopped and it had not actually  set its brake hold?    Not happened to me  but could it be you stopped then the car slightly rolled back before the brake hold had time to activate  - ?   Could it be you rolled to a stop due to an uphill incline, but  didnt actually brake to a stop .  Just ideas. I dont know if any of them might actually apply or fit the circumstances.

 If its come to the point where you cant explain it or trust it, it may be advisable to consult the dealer.

Never happened to me. Are you absolutely sure your foot wasn't hovering over the accelerator (and just touched it a very tiny little bit)?

No chance - foot hovers over brake pedal.

Brake hold will only work if the vehicle is absolutely still.

Absolutely completely stopped.  It tends to happen when, for example stationary at traffic lights, it decides it must be time to go... so it goes!  Oh, and no loose carpet... I have rubber mats.

Have handbrake auto application set to come on after power off.
Where in Settings is the 'Handbrake Auto Application' to be found? I couldn't find any reference to it in the manual.

I never use brake hold. Many automatics dont have this, electric handbrake is all you need and it releases automatically when you pull away.

See manual page 501: Automatic Brake Hold "Braking is kept for up to 10 minutes."

In my experience, "a while" is a bit shorter than ten minutes. In start/stop traffic, so when I use brake hold, I (fortunately) never have to wait that long before everything starts moving again (a bit).
Sometimes we had to wait a long time for example at a railway crossing (without measuring time). Brake hold never released spontaneously.

I would seriously test how long brake hold keeps and if that is considerably shorter than 10 minutes: to the dealer!
If nothing goes right, go left!

Hmmm, brake hold auto releases after xx minutes !

Sounds dangerous to me with the proliferation of drivers texting when stopped.

I've never been stopped that long  on brake hold  but IIRC it doesnt just let you loose to roll.  It applies the handbrake instead .

Speaking as the driver behind you at the traffic lights, I would much prefer you to use the parking brake rather than brake hold. It shows consideration for other road users, and it’s hardly difficult.

As Colin B mentions in his post before you, it is about being considerate to the driver behind you and not dazzling them with your brake lights. If you use brake hold, they're on... even if you then apply the handbrake, the break hold is still on and brake lights illuminated.

...I think that some drivers in cars with manual gears and a normal handbrake still sit with their foot on the brakes and dazzle people behind.
